| MINNESOTA | RN Case Manager: Mary T. Inc. has a 30 hour/week opening for an RN Case Manager to work with our group homes serving persons with disabilities. The successful candidate will have prior group home or disability-related experience, attention to detail, a willingness to travel to various programs, and the ability to work independently. Duties include: ensuring quality care for the persons who reside in our programs, supervision of LPN services in the homes, assisting with medical issues, interaction with other medical professionals, and some on-call responsibilities. This position offers flexibility in work hours and is benefit eligible. Lead Staff: Ensures the safety, well-being, and good care of all the clients. Under the direction of a home supervisor, orients and trains new staff, provides work direction to staff on duty, performs direct care duties, may do program writing and administrative work. The successful applicant will have at least 2 years of group home direct care experience as well as a 2-4 year post-secondary degree. Requirements include: strong verbal and written communication skills, proven leadership ability, and attention to detail. Must be flexible with AM and PM shifts with every other weekend
Direct Care Support: Responsible for the direct care, safety, and well being of the clients; the complete assigned programming responsibilities and care plans; assisting the client in the achievement of physical and emotional comfort; and promoting and encouraging client participation in appropriate activities both in-home and in the community.
