of our Home Health Care.

Our home health care professionals know how much people value being in the comfort of their own home. We hear how important it is to feel safe and in control.

Under the direction of your physician, we will work closely with you and your family to develop a personalized plan of care to meet your needs. We will help you recover from surgery, illness or injury and rebuild your independence at home.

Feel Right at Home with high quality services ranging from light housekeeping two hours per week to 24 hour live in care seven days per week. Our caring staff is highly skilled and trained. Mary T. Home Health is Medicare certified and a Minnesota licensed agency serving residents of the Twin Cities and surrounding suburbs.

Skilled Nursing Services

All skilled nursing visits are provided by an RN or LPN. These
experienced professionals assist you in regaining your independence
and maintaining optimal health.

We provide individual home assessments, including safety evaluations, falls assessment, case management, and medication management. We provide quality care for your post-surgery needs, cardiac conditions, stroke, diabetes, wounds, and other chronic illnesses. We are here to help!

Therapies

Physical

Our Registered Physical Therapists will work with you to re-gain your strength and mobility. They will complete a safety assessment of your home and evaluate your equipment needs.

Occupational

Our Occupational Therapists help improve memory, attention, and motor function to improve quality of and independence at home.

Speech

We'll help you make progress with language, speech, swallowing, and cognitive abilities.

Medical Social Workers

Medical Social Workers assist in finding community resources, coordinating daily care conferences, and helping you with long-range planning.

Home Health Aides

Our home health aides assist in personal care needs such as bathing, grooming, dressing, range of motion exercises and more.

Homemaker Services

We provide light housekeeping, laundry, and meal preparation.
